We may disclose information when law requires it, including suspected abuse or neglect, and when there is a serious and imminent threat to your safety or the safety of others.

Your rights

You have the right to inspect and copy your record, to request an amendment, to request restrictions on certain disclosures, to receive an accounting of disclosures, and to receive a paper copy of this notice.

You may file a complaint with us or with the United States Department of Health and Human Services. We will never retaliate for a complaint.
